Default Code Help - P0161

I pulled into the parking lot at home last night after work and the check engine light comes on. Pulled the codes and P0161 H C came up- Heated Oxygen Sensor (HO2S) Heater Circuit Bank 2 Sensor 2-B. What is this and what does it mean? I believe the h means history and c means current? Car drives and sounds the same. Mods are cai, long tubes, x pipe, magnaflow catback. Any help would be greatly appreciated as this is my dd and hoping it is something simple.
Thanks.

You say you have long tubes and an x pipe, are you running cats, if not its probably a bad o2 simulator, or if you are a bad o2 sensor.

nope, no cats...when running codes, this code comes up as a history code as well, if that means anything else

Easy, Just plugs in to the harness where the right rear 02 sensor would plug in.
