Did you have your car tuned after the install?

Do you have scan tool software?

It would be easy to trouble shoot if you had scan tool to monitor HO2s.
Any way read this link.http://www.gearchatter.com/viewtopic10479.php
sounds like a damaged sensor

********UPDATE************
Today I found out that on a 2004 C5 you must have all 4 02 sensors connected, On earlier models you only have to move the rear 02 sensors to the front connections, So I hope this will help anyone else who does the Longtube with metric cats.

Now will have to wait for the extended connectors to get here

Also a note on the driverside 02 sensor it has a male connector going into a female, on the passenger side the 02 sensor has a female going into a male connector, and both rear 02 sensors both have females going into males. Anyone with another 2004 have this same setup ?

I had the same issues with the o2's, the late production 04's have a different plug than earlier C5's from what i was told by my tuner.
It may have been an anticipated change for the C6 05' model year and the vendor supplying them.

Let me guess, sounds like you bought LG motor sports LT headers

Only reason I am guessing this is because they had to send me the
different harness that did not come in the box for my 2004
you must have all 4 02 sensors connected
Only true if running cats, I am willing to bet you will still get codes for
the rear O2 sensors and have to have them turned off if not already done.
I just realized you said you had a tune so they may have turned them off.
They told me on an 04 with or with out cats to still
plug the sensors up and I should be good. Well I went without cats and still got rear activity codes.
I believe others with Hi-flow cats on an 04 have had the same problem.

If you have LGM's LT headers this is the harness they will send.
Got mine the next day when I called them.

Well you hit the nail on the head, I hooked all four sensors up this evening, Was trying to do so and get home before 10PM, well I left 1 sensor wire undone and when I started the car up multi misfire code, Put it back on the rack looked everything over and yep there it was.

Plugged it in let it idle for a little bit all seemed fine. locked the shop up and drove it home. ( Of course after the multi misfire I had to hit the throttle a couple of times to clear it all about 8 miles to the house).

Upon arriving at the house I checked for any DTC and none showed up will keep my fingers crossed, but atleast I know now that someone else other than me has gone through this too!

I have the same problem with the P0135 code just came on today. All the connectors are connected and I still have this code. Could it be a bad 02 sensor ? By the way the cats have been on for three weeks now and this code show up today.

If your connectors and wires are good, it could be a bad sensor 1 bank 1.
That code and the cats are not related.
