The manufacturer, Pfadt, is reacting in a stand-up way and I am in no way throwing stones. I've had my coilovers for 23 months and other than the loose nut issue in the first week they have been great.
It appears that I snapped a hard part, the top of the shock shaft where it meets the bottom of the flange on-top of the spring. It's my right rear wheel and sounds very much like the loosening nut issue; problem is when I got in there I could see the separated shaft/flange and could see the only thing keeping the shock from falling against the spring is the ~1/8" adjusting rod that runs up through the shaft/upper bolt.
Curios if anyone else had this problem and what may have caused it. I don't recall hitting any potholes, curbs or anything that gave me any kind of jolt.

My friend had exactly the same failure last year, same brand, and just so happens.....it was the right rear
In their earlier version the top mounting nut had a tendency to loosen up, they have come out with a later version that fixed the problem - in my case since I have the earlier version I ended up having it double nutted and that solved the problem.

I believe mine is the first version, I'll know for sure when I see the replacement body. In any case apparently there is a later version that includes a small brass sleeve/ring that goes over the shock shaft near the upper flange to add strength to help prevent the shock shaft/upper flange from shearing. Mine definitely did not have the brass sleeve/ring.
