I didn't with my Melrose headers and exhaust but they use both the front and rear O2's with Random Tech hi flow cats. Friends have gotten codes with other brands.

I've helped friends to install at least 4 set of headers, with no codes. If you take off the factory system and install new header system correctly, there shouldn't be any problems. Codes usually appear due to crossover of O2 sensors or other components not connnected properly. If a code or check engine light appears, you first need to get the DIC code to determine what caused it. Could be a simple fix like incorrect electrical connections. Headers are a simple weekend install, take you time, and make sure that what you unplug gets plugged back in correctly. Some headers systems have deleted the front O2 sensors and only have rear O2 connections so a good tuner can correct this by modifying the software in your PCM.

I have the Dynatech system. Deleted the rear O2's and placed the rear sensors in the front locations per advice of many. I occasionally throw a Slow response Bank 1 sensor 1 code. It's common from what I'm told but I still question why only bank 1 and not on bank 2. I will replace the bank 1 sensor and see what happens. The code is easily reset with the DIC.
