There's a number of sources, but that item is model year specific.

I updated my post in order to utilize the factory pcv valve and added pictures.

I replaced my oil pressure sender on my '98 and noticed my PCV hose connections were cracked, split, and separated. On the PCV end someone had taped up the rubber connector with vinyl tape. How useless was that? I had the oil sender unit replaced the first week I bought the car and the previous owner paid for it back in 2005. Maybe that's when the pcv connection was taped and the other connections broken. Anyway I replaced the rubber parts with stuff I had laying around my garage. List included: right angle spark plug boot, 3/8" plastic T (bought a set of universal T's from Autozone), 3/8" hose, and 5/8" hose. The spark plug boot came from a small block chevy set I had laying around for spare parts for my other cars. The factory 3-way Y connector was replaced by the 3/8" plastic T with 3/8" hose to connect the inline ports of the T to the two longer factory plastic tubes. I installed the right angle spark plug boot on the middle T which brought it inline with the other connections and plugged the shorter plastic tube into the spark plug boot. This shorter tube goes to the passenger valve cover. At the PCV end; I cut a piece of 5/8" hose 1-3/4" long and pushed the larger end of the pcv valve into it. I pushed a piece of 3/8" hose into the other end of the 5/8" hose and cut it off flush. Now the pcv valve can connect to the plastic tubing going to the new T connection. The pcv connection is pretty snug and tight but I guess you could put a zip tie or hose clamp around the end that connects to the plastic tube. Mine was tight so I left it as shown in the second picture. The other end of the pcv valve slipped into the stock rubber hose going to the throttle body. On the driver's side valve cover the right angle grommet was still good but I think a right angle spark plug boot might work there. On the passenger side valve cover I used a short piece of 3/8" hose to connect it to the plastic pipe that went to the new T connection. Hope this posts helps someone else. Sure I'd like to buy a new one but I didn't want to wait and I think the one I made is more durable and will work fine. You really can't see the new T connection and everything else looks and works fine. You won't see the most obvious stuff with the engine covers on.
