O2 sensor Needs replaced
Bank 2 = passenger side
Sensor 1 is directly after exhaust manifolds/headers to maintain proper A/F ratio
Sensor 2 is after cats to confirm cat is working correctly
So yours is the drivers side sensor plugged in after the cat. Keep in mind the wiring crosses over, don't get them mixed up. Most people running aftermarket cats/headers/no cats have the secondary O2 sensors "tuned out".

- Defective Sensing element
- Defective/non working heaters.
- Defective/Damaged wiring and or connectors
- PCM reference voltage not working
Before you throw in new sensors, I strongly recommend obtaining a VOLT/OHM meter and making sure that you have proper heater voltages at the fuse test points and proper ground.

Notice that the voltage to the circuit is ONLY SUPPLIED when the IGNITION SWITCH is ON. Make sure that there is full battery foltage available at that fuse.
