Since I was having my '03 in the shop to replace all gaskets and seals due to oil leaks , the balancer was replaced also. At 145k miles. No problems before with the balancer, just oil leaks.

Did mine after the first season of ownership, 54k and a slight wobble and no leaks.
Slight wobble didn't sit well with me. I did the balancer, ARP bolt, front seal, all new pulleys & tensioners and belts.
Peace of mind & quiet/smooth running where the work was done

When I did mine, I did all the major stuff when you get that far in: Ls2 timing chain, anti chain lash insert, Edlebrock 2 piece timing chain cover (allows you replace the cam and the other stuff without dropping the rack next time), ATI super damper (comes as a 3 peice so you dont have to pull the whole thing if it starts to go bad next time), ARP Bolt, belts,seals, Melling oil pump, might also consider your water pump while you are at it, 160 degree thermostat, etc. Pulleys and tensioners as said above. The PIA is dropping the rack, and getting HB off and on, so do as much as you can once you have it out..just my 2 cents.

I agree with all of that. Except oil pump and timing chain may be a bit overboard.

Default balancer and oil leaks

Oil leaks are what cause the rubber between the inner and the outer to fail. If you have oil on the balancer you should definitely replace it.
