Throw out bearing





My transmission started to make a weird noise in first gear only, when under stress - ie... starting on a steep hill.
The dealer ended up replacing the throw out bearing, the clutch and the flywheel, telling me the throw out bearing had come apart and damaged everything else. I was told perhaps there was not enough lube used upon manufacture. The car is 2 1/2 years old and has 16,000 miles on it. I don't race it or drive it hard too often. I've had manual trannies all my life and I've never had one go bad before, so I don't think it had anything to do with how I drive a stick.
Can anyone explain this to me?
