First you will need to download the Torque Pro App on your phone then purchase yourself an OBDII Bluetooth adapter. There is a range of OBDII bluetooth devices you can buy. I did a little research and the OBDLink MX seemed to be one of the best, but it's a little pricey. You can get the made in China ones on eBay for pretty cheap. I like the OBDLink because it comes with a 3 year warranty and 90 day money back guarantee.
