Harmonic Balancer Bolt
At the very least:
- what was the frequency of bolt replacement in term of time/miles between each bolt change? Did these three bolt changes happen over the span of many miles/years, or was it all in one weekend?
- Did the same mechanic do the same thing - i.e. Use blue Locktite - on each occasion? You are aware that the very definition of madness (and, perhaps, stupidity) is doing the same thing over and over and expecting a different result, right?

- was it a GM bolt? If so, was it new? If so, it comes coated with a sealant under the head from GM, and it's a torque to yield bolt, so I'm not sure if you're supposed to use any Locktite. I would still use it though;
- was it torqued properly?
- I have always only used the red locktite on any harmonic balancer (it's actually a dampener, not a balancer) because the job of that part is to absorb millions of harmonic vibrations from the crankshaft during the life span. If anything needs red locktite, it's that bolt.
