Default Rear Wheel Hub Replacement

Hi guys. I changed my passenger side rear wheel bearing. As I reinstalled the knuckle, and then installed the new axle nut, that's when I found this problem.

I reviewed a few videos regarding the job and found that blocking the opposite side wheel is a good technique to lock the limited slip for removing the axle nut. After I installed the new nut, and got it tight, I blocked the opposite wheel again to achieve the proper torque on the new nut. Ensuring the wheel didn't move, the axle shaft did still spin on the side I'm tightening, before I could get to the 160 lb/ft.

I reviewed posts and couldn't find a discussion on what this could be. Has anyone had this happen to them? Does it look like the limited slip is not holding? Am I doing something wrong?

Could I get y'all's expert advice?

I have always tightened the nut with the car on the ground.With it being limited slip I would think your issue is normal.
