Why? Can you not go down to your local Chevy dealer and ask for it? IIRC, you can talk to the service advisor and they can print out one for you.
I did that this week during an oil change. The Service Consultant has been there a long time and gave me mine. It showed the dates I came in and did not show me what work had been done.

So you came in and did nothing? That sounds odd. In this day and age, they should be able to see everything they did. So what happened? You went in, paid money and did nothing? I'd try and find another appointment clerk and ask them to do one for me.

appointment clerk?? the service rep can only print what info is available!!
i made an appointment for the battery, asked the service rep to print 'em when i picked up the car!!
now if you don't have a service rep, no guarantee that any/everyone of 'em will do it for you!!
i'd have to look what beyond both build sheets they were able to print!!
Maybe you are right. There is one more thing the appointment clerk can do besides make an appointment for my car to get fixed by the tech of my choice.
