The document is (known as) an FS-67. It is not actually a form. I gave the person I spoke with the particulars - that is my new address, my plate number and a few other details. There is a custom plate division and that's who you need to speak with. Good luck (by the way...spoke with NY DMV on Friday and got the document today)!

Florida's standard antique plate is blue. They have three red specialty plates for custom vehicles, horseless carriages and street rods. The one shown on the Cadillac, which is a beautiful car, btw, is the one for a custom vehicle. We carry all of them in stock except for the horseless carriage.
The $400 fee referred to is a new fee on any set of new wheels put on Florida roads. If you bring a car from out of state, you pay it. If you buy a car from a Florida dealer and are not transferring a Florida tag, you pay it.
One thing that Florida does that is actually kind of cool is that the state will allow you, under certain circumstances, to "certify" an old Florida license plate, which allows you to use it on your car.
