That is the list of "optional equipment" installed in your car at the factory. Don't know what the other designations are ("2V", "2P"), but the option codes are as follows:

Too bad there's not a little more of your sheet left that would tie it to a particular car.
If you look at the codes you'll see that some of the codes became so well known that we now refer to the option by the code.... for example: we all know what M40 and J50 refer to.
This is even more true with engines since we refer to the engines by their codes too... LT!, L68, L46, etc..
Do you recognize anything in those 2 strings of numbers that are printed in reverse on the bottom right that might tie the sheet to the car?
The loose piece you have placed at the lower left news to move straight up to the top... you can see where that little tab on that piece fits in the notch in the upper piece.

Spring codes may not necessarily indicate optional suspension. They're listed to tell the worker which springs to pick from the bins and install in the chassis.
The two typed numbers on the backside of the sheet are the most interesting. Could be key codes and a partial VIN. The first one looks like 501253. Can't see the second one well enough to read it.

The tank cleaned up nice with a coat of zinc paint and new hoses
Wow, the part that was left was the options list? That is some pretty good luck. Where'd you get the hoses and bolts from for the vapor separator? Those clamps look very green.
