Do I have to expand the flanges on the muffler to stop the rattles?
Unless the collector slides all the way into the pipe captures the insert it is going to rattle. Tweaking will help but when the insert slides slightly forward it will start rattling. I've had three different inserts and they all rattled no matter how much I tweaked them. I finally drilled a pilot hole in the bottom of the tube, just aft of the bell shape of the insert, and used a stainless, self tapping sheet metal screw to secure them. The screw holds the insert in place and I have driven probably 5000 miles with no problem.

Happy you said that, I thought about that but was reluctant to drill thru the chrome and maybe start an area of rust. Did you use or two screws per side?

I did this too, but toward the rear of the side tube on the side toward the frame, cant be seen unless it's on a lift.

I used a 3/4" screw. Not sure if it was a #8 but used a 5/16" hex wrench. The screw pictured is 1"

Just to show the placement. You can put it in the very bottom like I did or around towards the inside. Mine does not rattle with one screw but if it did I would not hesitate to put a second one in towards the rear. When you tighten the screw, snug it so that it dimples the tube a little bit. That will help keep it tight as it is pulling against the insert.
