I have a squeak or chirp coming from the drivers side rear wheel.
The faster I go, the quicker the chirp/squeak. This occurs regardless if I put on the brakes. However, when i apply the brakes, the chirp/squeak sound is more prevalent. It has rear disk brakes.
I replaced the leaf spring and bushings; however, the chirp/squeak is still there.

Most likely one of the u-joints: cheap and relatively easy to replace.
IF NOT:
Could be one of the wheel bearings (inner/outer): not overly expensive but considerably more difficult to replace.
Should i just replace the U-joint only, or is it wise to replace the entire axel? Also, if you replace one side, is it recommended to replace the other?

There are 2 u-joints per halfshaft so 4 u-joints total if you want to do both sides at the same time.
I'd minimum replace both on the squeak side. Generally, if the u-joint is bad you'll know it when you take it apart (rust, dust, dry, worn, etc).
While you have the halfshaft out you can check the axle.
