Anything can do that but I doubt it was a faulty crank.
Seized bearings
dropped valve
seized drivetrain
serious overheat
bad balancing

I have put 18#s more then once to my stock crank. The picture certainly makes me wonder what would do this to the crank...unless it was somehow a weak crank from the factory or reused crank that wasn't mag'd and inspected and had a problem.
