The hole in TB is an old-skool trick that was used to get cars with big cams to idle before good tuning software existed, and/or before people figured out how to adjust for it.
With today's advanced tuning tools like EFI-Live, HP Tuners, etc, there is no need whatsoever to drill the TB blade.
I'm not sure what difference popping the hood makes...does your car stall out, not want to idle? Or are you just seeing the motor rock back and forth a bit? (it will always do that).
If you are having problems keeping it running, then get it to a good tuner...just because you had it tuned, doesn't mean it's perfect.
