Ignition Switch Problem?
My '04 (28,000 miles) starts and runs fine when cold. It also runs fine when warm and hot. However, on warm restarts, the engine will turn over properly and start, but then die immediately. This happens anywhere from two to four times. Sometimes, depressing the accelerator to the floor and immediately releasing it prior to starting does the trick. After the problematic start, it runs flawlessly and with full power. I don't seem to have any other issues related to an ignition switch problem, or any other problem for that matter.
Any thoughts? Thank you!

No other codes were thrown after the failed start-run sequence. Car still runs flawlessly once I can keep it running.

first code is speedometer signal circuit malfunction
second code is right actuator feedback open
My car is doing a similar thing and just tonite it shut off while driving for the first time. Had just been a startup problem, fuel pump not running after car starts and not running prestart cylce.
Car will stay running if I hold the key in the crank position.
I changed out the ignition switch,,,,,no deal.
cleaned a few ground location I could get to,,,,no deal
tonite the car started and ran strong so I took her for a spin. While cruising down the highway she died. I got her restarted and got her home.
I pulled the codes and are as follows:
60-IBC B0521H - Tachometer signal circuit malfunction
first time for this one tonite
BO-RFA U1096H - Lost communication with XXX
BO-RFA U1064H - Lost communication with XXX
BO-RFA U1016H - Lost communication with XXX
10-PCM P1571H - Traction control torque request circuit
I am at wits end as I am sure you are too!
Do you get gauge and dash lite function when it dies? I did until tonite

If you haven't had problems with the codes you have getting thrown before, I'd suspect you've got a ground that went out on you somewhere. My speedom. code cleared and hasn't returned, but the HVAC one did. Funny thing is, I'm having no problems with my climate control.

I agree, I think is a ground prob!
Check your pm's in a minute for my phone number,,,,I would like to compare notes

Check two things:
1). There is a large ground connector to the left side (front) of the block on the left frame rail. The connector is designed as an integral socket and plug with four (?) wires going into it. Unplug and then re-insert the plug into the connector. Even if the terminal ring is tightly bolted to the frame rail, if the connector contacts are oxidized, it could cause the problem.
2). Unplug and then re-install the IGN relay (#42) in the main fuse block. I think it might stick sometimes.
I just played around with those two "obvious" things, bending the wires and jiggling them as they enter the ground connector and the unplugging/plugging in the IGN relay. It clicked when I removed it (key off, battery connected), but did nothing when I plugged it back in.
I have not had problem one today, whatsoever. This is with probably 8 - 10 warm/hot restarts (ran a lot of errands today). Car ran flawlessly. When I work on mine, I am going to pull every ground connector plug and clean the contacts. This is on top of cleaning each ground contact where they mate to the frame.
Hope this is it and it works for you!
Good Luck

Well I called in the Air Force and they came through, three fellow non club clubber's, one of whom is a jet tech talked with yet another non club club on the phone after we eliminated several more grounds and the starter connections as the problem. He pulled the fuel pump relay and jumped it, fuel pump came on and stayed on car ran great enough for 13.3 1/4 mile tonite at 108 mph. The PCM is due to be replaced Monday. Hopefully problem soon to be solved!
