Driver-side valvecover pcv hole
Is there an adapter that will push into the pcv port the same way the stock line does, but have a male tube fitting? It doesn't look like the stock one will come apart from the hose assembly easily being formed plastic...

The stock hose wraps around the back of the intake then to the passenger side and up to the front. If your's is cracked or broken, you'll have to buy the whole thing. I think it runs like $99 or it use too a couple of years ago. Check with Gene Culley for the current prices.

I got mine apart with a hair dryer. Warmed it just enough to make if flexible. I then used a longer rubber hose to the "Y" fitting behind the intake. This was done to remove the interference the shorter hose had with the FAST. If that hose is truly $99, I think the 04 valley cover is less then that and would eliminate the hoses altogether. That is how mine is now setup.
