There are actuators which might be the issue along with the vacuum line that feeds everything from the engine. I would suggest you start by pulling the codes and see what is reporting first. Then take a good look at the vacuum line in the engine compartment coming from the rear of the intake manifold near the MAP sensor, down the left behind the battery and into the firewall, looking for a crack or dissolved from battery acid issues.
Would agree. The drivers actuator door is probably in the right position for A/C cold air but not moving to the position to deliver the heated air. I would suspect a broken gear in the drivers side actuator. Would also think you should be throwing an actuator code. Do the modes change the vents properly? If so it is not a vacuum leak. Check your codes...

Vacuum controls DAMPER doors which changes the vents that the air exits not TEMPATURE. You have an ACTUATOR problem. Read and post your codes. That will prove it if its completly dead. If the gear is busted, it may or may not throw a DTC.
Like previously stated, its most likely a broken drive gear as can be seen in this picture. Common issue:
You can get a mirror and examine the actuator and see if it moving when the temp is adjusted. Just examine the center hub of the actuator:
