Anyone have trouble with a harmonic balancer coming loose? If so, what was the solution?

This is common. Do a search. Replacement is your only option if you are talking the outer portion of the balancer. If it is loose on the crankshaft, someone may have reused the attachment bolt. The bolt needs to be replaced and properly torque.
If it has spun on the hub then you should likely pull-it and replace it with an aftermarket unit so you can pin the crank. Pinning and a properly torqued new bolt should work even if the press-fit is weak.
You need a new crank if the crank snout is worn to the point that a new balancer is a slip fit.
You do need to check because the most common failure of a stock unit is the outer ring slipping off the inner hub.
