I had this same problem with these same headers, even the same sensor. The cuprit was an O2 sensor wire that worked its way too close to the exhaust on a long trip and had become damaged. I had the wire fixed and the codes went away on their own about a day later.

Finally figured out the problem. Was a bad 02 extension. Dynatech said they had a vendor who sent them some bad extensions. They sent me one and I've had no more problems to date
