frames rust from the inside out, there is nothing you can do short of removal and sending it to readi strip for dipping, then dipping it into a big pool of paint to coat it inside out...
of course this is impractical... some do it though..
you can fix it cosmetically buy sanding and painting...
It depends on what you are trying to accomplish... If you have no 'serious' rust problem with the frame and just want to improve the appearance, a power drill with wire wheel will knock off the loose stuff, then POR-15 products can be used to paint over [and convert] the remaining surface rust to inert condition. There is no really good reason to remove any remaining surface rust with this approach.
If you want to re-do the frame to 'new' status...or better...you'd have to disassemble to a bare frame and do the dip/strip/blast/whatever approach.

That's what I did on my 71'. The part you see looks good, but you can't get to the top of the frame so if you want to do a NCRS job the body needs to come off.

I'm replying to joe's post.
While I believe that frames rust on the inside too, I don't think that the surface rust you see on the outside of frames started on the inside of the frame.
I believe that frames with serious rust through, have problems that may have started from the inside and often because of clogged drain holes.

I sandblasted my frame when the body was off , then epoxy primed and painted with acrylic enamel. For the inside , I blew it out with a wand to get all of the sand and debris out and rustproofed the inside with a wand run through the frame. I will also repeat the inside process periodically even though the car never sees bad weather.
that sounds like the way to go but I really dont want to take the body off because its still in pretty good shape, I just want to freshen it up over the winter to make sure it stays that way,
I am in the middle of my frame up resto. I am going to sandblast my frame and paint it.
I am now doing all the other smaller parts testing out different ways. Hopefully, over the christmas holidays I will has the frame completed. I will see. Still a lot of work to do.
